Students will only be administered medication if there is a signed consent form on file for the student for this specific medication. 

Please note: A new consent form is required for each different type of medication. Consent forms are only valid for the Academic year during which they are submitted - new consent forms will be required at the start of each Academic year. A physical written signature is required - electronic signatures or verbal consent are not accepted. Forms must be fully completed and medication provided in the original packaging.
Medication will not be administered if the above requirements are not met.
